Chronic Disease Self-Management Program

The Chronic Disease Self-Management Program is an evidence-based intervention developed and refined by the Stanford University Patient Education Research Center since the late 70s. The program crosscuts all chronic health conditions and is offered at no cost to participants in Utah and most other states around the country. 

As Diabetes Education Programs look for ways to provide patient self-management support options, this Forum exists to introduce CDSMP as a supplement to current diabetes and general disease management approaches. The structure and methods of CDSMP will be described as well as the published outcomes and how to engage patients in this program.
